Survey tools can be used at little to no cost. There are multiple online websites that will help you formulate a survey and disseminate it via email or print them up to send out hard copies. I have 3 sites that I have used in the past and have great tools on the back end of them to really show the results of your survey in a professional manner.
This is a free site, but you can uprgrade your account. It has many back end tools to compare data on all questions, demopgraphics and answers. You can tie in multiple responses to see how they felt about a particular question.
This is a free site, but you can uprgrade your account. This seems to be a very popular site. Many work, company and personal surveys can be done via this site. They have the ability to do the survey for you and create everything for a fee or you can do it all yourself.
This is a free site, but you have to uprgrade your account to get access to real analyzing tools. This is a great site for your smaller children and things that will not be so intricate, unless you want to upgrade. This site does have a free web survey creator tool.
